Credential Request Form

Please complete this form in its entirety. It can be filled out individually or on behalf of other members of your department. This form is intended to request a Single game Credential for Media, or Visitors. Please note that submitting this form does not guarantee approval. You will be notified via email regarding the status of your request. If your credential is approved, you will receive further instructions on when and where to pick it up.

Disclaimer and Agreement of Terms of Use for Credential Request

This credential serves as identification and grants entry to events or facilities owned or operated by Utah Tech University. It must be worn and clearly visible at all times while inside the event or facility. The bearer of this credential agrees to comply with all Utah Tech University and NCAA rules, regulations, and policies.

This credential is strictly for the individual to whom it is issued and is not to be shared. In the event the credential is lost or stolen, it must be reported immediately along with an explanation. Any misuse, including sharing or unauthorized use, will result in immediate ejection from the facility and revocation of the credential. Replacement credentials will not be issued.

Utah Tech Athletics, Utah Tech Event Services, or NCAA representatives reserve the right to revoke credentials at any time, at their sole discretion. This credential provides access only to the specified locations within the facilities as designated and only during the designated event.

By accepting this credential, the bearer agrees to adhere to all stated conditions and assumes all risks and liabilities associated with its use. This credential is non-transferable.
